 Intention Holistic Health PLLC is a Texas-registered Professional Limited Liability Company (PLLC).

Clinical services are performed by Alicia Harrison, MSN, APRN, FNP-C, licensed in the Commonwealth of Kentucky and state of Texas.

Business operations follow Texas law; clinical care follows Kentucky law and telehealth regulations. Clinical care is not provided in Texas.

2. INFORMATION WE COLLECT 

We collect two categories of information:

  • Protected Health Information (PHI) (clinical services only)

  • HIPAA-compliant telehealth platforms
  • Clinical intake forms and consent documents
  • Forms and documents you upload inside Practice Better 

PHI must not be submitted through:

  • Website forms
  • Email
  • Text message
  • Kajabi
  • Social media messaging

 Submitting PHI outside secure systems may risk unauthorized exposure.

 Clinical patients receive a separate HIPAA Notice of Privacy Practices (NPP).

3. HOW WE USE YOUR INFORMATION

 3.1 To Provide Coaching or Website Services (Non-PHI)

 We may use personal information to:

  • Respond to inquiries
  • Provide non-clinical coaching services
  • Deliver digital products
  • Send newsletters or marketing emails

 3.2 To Provide Clinical Services (PHI)

 Provide clinical nurse practitioner services

 We do not sell PHI or use it for marketing.

If we learn we have collected personal information from a minor without parental consent, we will delete it promptly.

 

7. YOUR RIGHTS (Clinical Patients)

 If you become a clinical patient, you have rights under HIPAA, including:

  • Requesting corrections
  • Requesting restrictions on disclosures
  • Requesting confidential communications

 These rights are described in your HIPAA NPP.

 8. YOUR RIGHTS (General Website Users)

 You may:

  •  Opt out of marketing emails
  • Request access to or deletion of your personal information (non-PHI)
  • Update your contact information
